How to obtain your patient records

Following Dr. Burkhardt’s retirement, you are able to obtain a copy of your patient records in the below ways.

Once the office is closed, your medical records will be maintained by our office until December 31, 2022.

Access your records using our Patient Portal

Click on this link to access your records using your unique password.  If you don’t know your username, please contact our office. If you have your username but don’t know your password, once you enter the Patient Portal face page, it will allow you to reset your password.

Transfer your records to another doctor or pick them up at our office.

If you require records to be mailed to another eye doctor or wish to pick them up at the office, you must:
