Dr. Burkhardt’s Retirement and Pending Office Closure
I am writing this letter to inform you that I will no longer be able to serve as your eye care practitioner at my practice in Chilton. After 28+ years I have decided to retire, and unfortunately, no one to date has chosen to take over my practice or obtain its records. At this time I will continue to see patients, but unless someone else comes along, it looks like eventually, I will need to close my practice.
The last day that I will see patients will likely be sometime in March 2022. I will continue to provide vision and medical care for you prior to this closing date, as well as provide eyeglasses and contact lenses. However, at any point in time please select another optometrist to continue your care. If you have insurance, you may call your insurance provider for assistance in selecting another eye doctor. To obtain a copy of your records, please use the patient portal on this website. If you require records to be mailed to another eye doctor or wish to pick them up at the office, records may only be released with a SIGNED release form from the patient/guardian (a link is located on this website to print and complete the form). Requests will be filled as soon as possible but please allow 2-4 weeks for their transfer.
Once the office is closed, your medical records will be maintained by our office until December 31, 2022. Until then you may contact our office for further details on any future medical record requests if you are not able to access the Online Patient Portal.
Thank you for trusting me with your health care needs throughout my time here in Chilton. It has been a pleasure caring for everyone, getting to know my patients personally, watching families grow and children becoming adults – I leave with many bittersweet memories. I wish you continued health and happiness in the future.
